Phoenix Health Therapy has signed a Business Associates Agreement (BAA) with the vendors that power both our EHR and Operations Management System to ensure they appropriately safeguard the PHI they receive.

We retain your information as long as necessary to fulfill the purposes outlined in this Statement, unless a longer retention period is required or allowed by law.
